Congrats on joining the COV team! There are multiple steps that must be completed prior to your official first day:
- You will have to complete two forms: Applicant Request to Release Information form and SSA-1945 form. These forms may have been signed at your interview or HR will contact you for preboarding.
- Once you have completed both forms, HR will email you an itinerary of the date, time and location of your pre-employment appointments.
- Each position requires both a medical and livescan.
- For your medical appointment you must bring a valid ID, if you are a minor you must have a parent/guardian present.
- For your Livescan you must bring the completed waiver (emailed prior to you by HR), a valid CA driver's license or ID, and if you are a minor a parent/guardian and completed parent/guardian waiver (emailed to you by HR).
- Your hiring is contingent on sucessful completion of both the Livescan and medical.
- Once you have successfully cleared both the medical and the Livescan, the Department will contact you for a start date. You should not resign from your current position until you have the actual start date.
